Update on CPC+model

Medicare partners with state and commercial health plans to transform primary care delivery.
The CMS announcement today provides details on a new initiative – the Comprehensive Primary Care Plus (CPC+) model which partners together CMS, state Medicaid agencies and commercial insurance plans across 20 regions. Physician practices who join will participate in one of two tracks and can receive upfront or performance based incentive payments. CMS and other payers will provide prospective monthly care management fees (CMF)s according to beneficiary risk tiers.
The program requirements ensure that practices in each track will be able to build capabilities and care processes to deliver better care, which will result in a healthier patient population. Payment redesign will offer the ability for greater cash flow and flexibility for primary care practices to deliver high quality, whole-person, patient-centered care and lower the use of unnecessary services that drive total costs of care.” CMS plans to enter into MOU’s with payers after which primary care practices will have an opportunity to apply and join the program.
